http://www.ntuvmdc.net/custom_99471.html 家禽流行性感冒診斷 家禽流行性感冒診斷 家禽流行性感冒診斷王儷蒨臨床暨分子醫學研究室 (獸醫二館702室)國立臺灣大學獸醫專業學院 背景介紹    家禽流行性感冒為高度傳染性的疾病,由A型流行性感冒病毒所致。A型流行性感冒的亞型由病毒表面的兩種醣蛋白來決定,即血球凝集素(hemagglutinin, HA)蛋白及神經氨酸酶(neuraminidase, NA)蛋白,目前已知有18種HA亞型及11種NA亞型。除了H17及H18是來自蝙蝠外,其他所有的亞型都可以在水禽中發現,故水禽被認為是A型流行性感冒病毒的原始保毒動物(reservoir)[1]。分支2.3.4.4(clade 2.3.4.4)的新型高病原性禽流感病毒(包括H5N2, H5N3 及 H5N8)造成2015年台灣的陸禽與水禽禽流感疫情的爆發,導致巨大的商業損失[2,3] 。根據臺灣行政院農委會動植物防疫檢疫局的官方資料,此新型禽流感病毒的HA基因與2014年韓國鴨隻分離到的H5N8病毒十分接近,病毒其他的基因則與中國、日本與韓國的禽流感病毒相近[4]。因此推論臺灣的新型禽流感病毒是來自國際候鳥傳播而非來自本土病毒的演化。    自1970年代,H6N1禽流感病毒就經常在臺灣雞隻分離到[5],且已逐步在地化並造成雞隻的潛伏性感染。於2013年全世界首件人感染H6N1[6]及狗自然感染H6N1病毒的案例[7]在臺灣被報導。這些事件顯示H6N1病毒在臺灣的廣泛性及衝擊性。禽流感H7N9病毒於2013年的中國,首次報導造成人類的感染,病毒是源自於家鴨與雞隻的病毒經多重物種間的傳染及基因的重組所造成[8]。這引起了鄰近國家包括臺灣的高度重視,因為H7N9病毒可能藉由候鳥進行傳播。 實驗室診斷   現行A型流行性感冒病毒的診斷主要是藉由分子診斷的方式,增幅病毒的基質(matrix, M)基因[7]或是核蛋白(nucleoprotein, NP)基因[9]的高度保留區,來作為診斷。較傳統的方法,是利用反轉錄聚合酶連鎖反應(reverse transcription-polymerase chain reaction, RT-PCR),搭配電泳膠(agarose gel)的方式來呈像。目前較先進的方法,則是利用多重反轉錄聚合酶連鎖反應或是恆溫環狀擴增法(loop-mediated isothermal amplification, LAMP),搭配生物晶片(biochip),來作病毒的診斷與同步的病毒亞型鑑定[10] (圖一)。現行亦有利用多重反轉錄聚合酶重組酶增幅法(reverse transcription-polymerase recombinase amplification, RT-RPA),搭配毛細電泳(capillary electrophoresis, CE),來同步作病毒的診斷與亞型鑑定,其敏感度可達1個病毒複製數目(copy number)[9] (圖二)。 結論   由於A型流行性感冒病毒的人畜共同傳染性以及病毒高度的變異性,正確與高靈敏度的診斷是非常重要的。流感病毒的同步診斷與亞型鑑別亦為趨勢,有助於流行病學的追蹤與監測,此有賴於設備完善且操作經驗豐富的實驗室來診斷,方可獲得可信賴的檢測結果。 圖一、以恆溫環狀擴增法(loop-mediated isothermal amplification, LAMP)結合生物晶片(biochip),作病毒的診斷與同步的病毒亞型鑑定 圖二、以多重反轉錄聚合酶重組酶增幅法(reverse transcription-polymerase recombinase amplification, RT-RPA)結合毛細電泳(capillary electrophoresis, CE),同步作病毒的診斷與亞型鑑定 參考文獻1.  Webster RG, Shortridge KF, and Kawaoka Y.
